PROMPT1 (CAN)
Objective Data
1. Small stature
2. prominent belly
3. 8 percentile for weight and 2.9 percentile in height she is in the last percentile in the scale
Subjective Data
1. Tired and irritable till she gets rest
2. Painful belly gas
3. Throws up after eating bread, foul stools (loosing fat in stools)
Pertinent positive data
After the education on what Merci can eat, Mrs. Garcia understood the education and will most likely comply on the diet that has been prescribed and encouraged for Merci to eat. The only way that her symptoms will improve. If the diet is managed all her symptoms may resolve. Symptoms come and go, they are not permanent. Education on avoiding foods that contain gluten is key to help her with symptoms.
Pertinent negative data- Merci has a prominent belly and is on the last percentile in the scale for her age along with small stature. This may be due to her body not absorbing the proper nutrients. Symptoms of celiac disease include gas, swollen belly and bloating. According to Stanford Health Care, this happens because the small intestine can’t absorb nutrients from food. Merci might be at risk for failure to thrive if her weight and height do not continue to go up as other children her age. Her fatigue is also part of the disease.
PROMPT 2 (LORI)
The case study is about a pediatric patient who has been diagnosed with celiac disease when she was only 11 months of age. The examples for subjective data that has been presented at the video are the following: according to the mom, Merci complains of painful belly gas every day, her appetite is only good half of the time, she loves playing but she gets tired easily, she also wakes up at night, she vomits after eating bread, usually constipated but occasionally having diarrhea with a foul-smelling stool. On the other hand, the objective data shows that Merci has a physical appearance of being slim with a noticeable distended belly, she has a smaller physique than expected, and Merci’s age is a 30-month-old. Moreover, the pertinent positive data are the following: Merci has a history of celiac disease, she gained weight, but she remains in the last percentile for both dimensions, she has a noticeable distended belly. While the pertinent negative data is when the mother verbalized the concern of Merci might be risk for acquiring diabetes. According to the practitioner, every patient can experience different types of symptoms. Therefore, she will just closely monitor Merci by doing some lab studies, stool sample, diet modification, and a referral with nutritionist and gastroenterologist.
